Link to Model Release Form Example - Feel free to download and edit. Always get permission to use a person's likeness in your artwork even if they are doing it for fun, this helps avoid any issues or misunderstandings down the line. I've also had clients request model releases for corporate reasons. Reminder, that if you are paying your model more than $600 a year it is generally considered taxable and additional paperwork is required by both parties (please consult your accountant).
